How to find a new primary care doctor with an out-of-state plan (15 recorded calls)

A call4me user in Baltimore needed a new primary care doctor and had an out-of-state Blue Cross Blue Shield PPO. Their AI phone assistant called 10 practices. Here's who was taking new patients, who took the plan, how far out the first opening was, a $2,121 surprise, and all 15 recordings.

Finding a new primary care doctor, 15 calls to 10 practices, 2 new-patient visits booked, recorded

The short answer: how to find a new primary care doctor comes down to calling several practices and asking the same four questions at each one: are you taking new patients, do you take my insurance, what's your first new-patient opening, with anyone, and is there a membership fee. On 15 calls to 10 practices in and around Baltimore, it took an afternoon and a morning to get two new-patient visits booked, and both were about 5 and 6 weeks out.

What the calls turned up:

  1. Out-of-state Blue Cross Blue Shield worked everywhere that answered the question. The patient has a Blue Cross Blue Shield of Alabama PPO and lives in Maryland. Four practices said yes. One of those front desks first said "I don't believe we can because it's out of state," checked, and came back with "yes, we do take the insurance."
  2. The doctor you pick may not have room. One practice's phone menu says "We are currently not accepting new patients." At another, the only providers with new-patient openings were "the three female nurse practitioners." The number listed for one doctor rang a different practice.
  3. The fastest openings weren't where the visits got booked. Two practices offered a new-patient slot about a week out (October 8 and October 9). Neither got booked: one was at a different office and wasn't the slot the user had asked for, so our caller passed it along instead, and on the other call our caller went quiet. The two visits that did get booked are on November 3 and November 13.
  4. Ask about a membership fee. One office answered with "Dr. [name removed] is a concierge provider." The membership is $2,121 a year, and you start with a meet and greet, not a visit.
  5. Plan on more than one call per practice. Four calls ran into an 8-minute limit the user's agent had set, mid-conversation. At two practices, a second call finished what the first one started.

These calls were made by a call4me user's AI phone assistant on Wednesday, September 30, 2026, between about 3:25 and 4:03 pm Eastern, and Thursday, October 1, between about 8:30 and 9:52 am Eastern. The user is okay with us sharing them, and every call is below with its recording and transcript. In the audio and transcripts, we muted the patient's name, date of birth, address, phone numbers and insurance member and group numbers, plus every staff member's and clinician's name. The visits got booked at two practices that we describe but don't name, because naming them would say where this patient will be seen. We muted their names, their health system's name and their addresses in the recordings too. We also cut silences longer than about 3 seconds down to about a second. Conversation, hold music, hold messages and menus are all left in, and each cut is marked in the transcript. A 37-second test call the user placed to a personal contact isn't included.

Some quick counts, ours and not the practices': of the 10 practices, 4 said they take the out-of-state BCBS PPO (and a fifth registered the patient with it without saying either way), 1 said on its phone menu it isn't taking new patients, and 2 got a booked new-patient visit, 33 and 43 days after the second morning of calls.

Does Blue Cross Blue Shield work in other states?

For this patient, yes: every practice that answered the question said it takes his Alabama Blue Cross Blue Shield PPO in Maryland. The phrase that worked was naming the plan and "through the BlueCard program." At Sinai General Medicine Associates, that got "Yes, we do accept that insurance" on the first try. At Meshulam Medicine: "We do."

The BlueCard program is how Blue Cross Blue Shield plans work across state lines. Blue Cross Blue Shield's own glossary describes BlueCard PPO as a national program for members traveling or living outside their own plan's area, who get their plan's PPO level of benefits from providers designated as PPO providers there. It also lists a number for finding providers in another Blue company's area, 1-800-810-BLUE. Whether a given office is in network for your plan is still a question for your plan and the office. Ask both.

Front desks don't always know it, though. At East Baltimore Medical Center, the first answer was no:

"It has to be Maryland, DC, or Virginia. I don't believe we take out-of-state insurance, even though it's Blue Cross Blue Shield. But I'll ask."

A couple of minutes later: "Okay, so yes, we do take the insurance. It would just be out-of-state PPO." If you hear no, ask them to check. That call is below, with the other 14.

Primary care doctors accepting new patients: what "yes" sounded like

"Accepting new patients" came with conditions more often than not:

And at the Baltimore County practice, the plain version: "right now, Dr. [name removed] is accepting new patients."

A concierge practice charges a yearly membership on top of what your insurance covers. Meshulam Medicine takes the patient's plan and still charges the $2,121. Nothing on our call said what the membership includes, so ask.

The morning's first call to this practice got three times on November 3, but the 8-minute limit cut it off while the scheduler was asking which one. This call, about an hour later, picked one.

How to find a new primary care doctor, step by step

  1. Make a list of 8 to 10 practices, not one. It took 10 to get 2 bookings. One had stopped taking new patients, and one number didn't reach the doctor it was listed for.
  2. Get the number from the practice's own website. The number we had for Kingdom Medicine rang Primary Medicine, a different practice.
  3. Lead with "new patient." "I'm calling to set up a new patient appointment" got straight to scheduling. In phone menus, listen for a new-patient option. At East Baltimore Medical Center it took four recorded prompts to reach a person.
  4. Ask about insurance by name and add "through BlueCard" if your Blue Cross Blue Shield plan is from another state. If you hear "I don't believe we can," ask them to check.
  5. Ask "is there a membership fee?" before anything else at a practice you don't know. One answer was $2,121 a year.
  6. Ask for the first opening with any provider, not just one doctor. "Any weekday's fine, the earlier, the better" and "with any physician taking new patients" is how a nurse practitioner slot came up. Ask which office it's at.
  7. Have the registration answers ready. Every practice that registered the patient asked for some of: full name, date of birth, address, phone number, the insurance member ID and group number, and whose name the insurance is under. One asked for a Social Security number and was fine without it ("Absolutely, that's fine"). An automated system texted a code to the patient's phone to verify him.
  8. Pick a time on the call. Our caller tried to say goodbye before choosing one of three times. The scheduler stopped it: "you didn't tell me what time you wanted."
  9. Before you hang up, get: the date, time, provider, office address, and what to bring. Both practices said the same thing: arrive 15 minutes early, with your insurance card, photo ID and a list of your medications.

What to say: a script that worked

Hi, I'm calling for [patient name]. I'm hoping to register [him/her/them] as a new primary care patient and book the earliest weekday appointment you've got with any provider taking new patients.

When they ask about insurance: "[Plan name], [PPO/HMO], out of state through the BlueCard program." If they say no: "Could you please check once, just to be sure?"

Then: "Is there a membership fee? What's the first new-patient opening, and with which provider? Which office is it at? What should [he/she/they] bring, and how early should [he/she/they] arrive?"
