When an application needs a phone answer, your existing assistant can ask BWC. A real customer call clarified a paper route for an owner with an ITIN, without submitting the application.

Ohio workers compensation insurance, a real call about a blocked application

To apply for Ohio workers compensation insurance, start with the Ohio Bureau of Workers' Compensation (BWC) application for coverage, form U3. If an online field prevents you from completing it, ask BWC how to handle that field before substituting another person's information. A real call on October 5, 2026 clarified a paper application route for a customer whose foreign owner had an ITIN rather than a Social Security number.

The result was a specific next step: the representative said to use the paper application and enter the ITIN there. No application was submitted and no insurance coverage was issued during this call. The ITIN answer was advice from this representative about this situation, rather than an ITIN rule stated in the published form.

How to apply for Ohio workers compensation insurance

BWC's official application and instructions describe online, mail and in person submission. The instructions list a $120 nonrefundable application fee. For mail submissions, they require a completed, signed application with a check or money order. They also explain when coverage begins following a successful submission or receipt of the application and fee.

Prepare the business identity, contact information, owner or officer details, description of operations and estimated payroll requested by the form. Its owner information section labels the identity field as a Social Security number. The published instructions do not explain an ITIN alternative. That was the gap this customer needed BWC to answer.

For a blocked field, call 800 644 6292, the number printed on the application. Explain which field is failing, whose information it requests and the exact problem with the online submission. Ask for the correct submission route, required attachments and payment method. Keep the answer with your application records.

Ohio workers compensation coverage: what the call clarified

The customer's request was to find out how a foreign corporate owner without an SSN could complete the application. The online owner identity field was rejecting the ITIN. The call4me assistant reached a BWC representative and explained that obstacle.

The representative directed the caller to the paper PDF and said the ITIN could be entered on that application. The caller also checked how to submit it and pay the fee. That resolved the immediate question about what to try next. Acceptance of the completed application remained a separate step.

There was also a readback error: the assistant gave a callback number that differed from the one the customer had supplied. Both numbers are removed from the public recording and transcript. The application instructions were useful, but the call does not demonstrate accurate handling of every detail. Check any contact information given to a representative when reviewing a call.

What to ask when the online application rejects an identifier

Use a narrow question that a representative can answer:

I'm applying for Ohio workers compensation insurance. The owner is a foreign individual with an ITIN and no SSN. The online owner identity field rejects the ITIN. How should we complete that field, and which submission method should we use?

Then check the parts that determine whether you can act on the answer:

  1. Which form and section should we use? Distinguish the business tax ID from the owner's identity field.
  2. What identifier belongs in that field in our situation? Ask for clarification if the spoken answer differs from the printed label.
  3. Are additional documents needed? Get a specific answer for your application rather than assuming the phone explanation covers every case.
  4. How do we submit and pay? Confirm the destination and payment method for the route you choose.
  5. How will we know the application was accepted and coverage began? An answered phone question is not proof of coverage.

Have your existing assistant ask BWC about the application
