SimonMed scheduling phone number: the cardiac CT line, and what six calls about a CCTA with Cleerly found

A call4me customer's assistant called SimonMed Imaging six times to price, book and prepare for a self-pay coronary CT angiogram with Cleerly plaque analysis at Van Ness in San Francisco. Here are the two numbers, how fast each reached a person, what staff said about price, orders and prep, and all six recordings.

The short answer: for a heart scan, the SimonMed scheduling phone number to call is 1-844-615-2957. SimonMed's coronary CT angiogram page lists it under "Call to schedule" and says "For pricing information, please give us a call at 1-844-615-2957." A SimonMed representative called it "our direct number to the cardiology department." The general SimonMed Imaging number is 1-866-614-8555, shown at the top of every page on simonmed.com. The Van Ness location page lists 1005 Van Ness Ave., San Francisco, CA 94109, phone (415) 229-7825, fax (415) 229-7826, open Monday to Friday 7:00 AM to 5:30 PM. Nobody dialed the Van Ness local number on these calls.

How fast a person answered:

Between October 13 and 15, 2026, a call4me customer's assistant used call4me to ask about, book and confirm a self-pay coronary CT angiogram (CCTA) with Cleerly plaque analysis at SimonMed Van Ness. What SimonMed staff said:

The SimonMed phone tree, step by step

1-844-615-2957 (cardiology scheduling). "Thank you for calling SimonMed Imaging. Please hold for the next available agent." On call 1, a short burst of hold music, then a person at 0:13. Calls 2, 3 and 4 reached a person at 0:21, 0:20 and 0:19.

1-866-614-8555 (main line). As it played on October 15, 2026:

  1. "Thank you for calling SimonMed Imaging. Calls are recorded for quality and training purposes."
  2. The main menu: "For information on an existing appointment, press one. To schedule or reschedule your exam, two. Calls from physician's offices, three. For directions to our facilities, four. Para asistencia en español, oprima cinco. Medical records, six. If you are a patient and have a billing question, seven. For personal injury related billing questions, eight. For general information, zero. Medicaid patients, nine."
  3. A submenu: "If you need to schedule or reschedule an exam, press one. If you have already scheduled and need information on your exam, press two."
  4. An exam type menu: "For breast imaging, press one. Ultrasound or DEXA..." The prompt was cut off by the next step on both calls.
  5. Hold: "Please hold for the next available agent," about 30 seconds, then a message that starts "Did you know that the digital images SimonMed took today will be further processed..."
  6. A person, at 1:49 (call 5) and 1:44 (call 6).

Key presses don't show up in the recordings and weren't logged, so we can't tell you which digits our caller pressed. If your question is about a heart scan, 1-844-615-2957 skips all of this.

What to have ready

Listen to the calls

All six calls are published in full, with original voices and timing. Silences cover the patient's name, date of birth, address, phone, email, height and weight, the doctor's name and address, diagnosis codes, the assistant's name and staff first names. The procedure, prices, prep, CPT code and SimonMed's fax numbers stay audible. Nothing else is cut. They're also on the examples page.

0:00 Phone system: Thank you for calling SimonMed Imaging. Please hold for the next available agent.

0:07 AI agent: Sure.

[Hold music, about 0:08 to 0:13.]

0:13 Representative: [Unclear greeting, muted in case it included a staff name] How can I help you?

0:18 AI agent: Hi, thank you. What's your name, by the way? Sorry, I missed it.

0:22 Representative: [staff name muted]

0:23 AI agent: Great, [staff name muted]. My name is [assistant name muted], [customer name muted]'s assistant. This call may be recorded, but we're looking into a coronary CT angiogram with Cleerly analysis, and I was hoping to ask a few quick questions about availability and self-pay pricing.

0:39 Representative: Sure. What is his last name?

0:43 AI agent: [customer surname and spelling muted]

0:48 Representative: And the date of birth?

0:51 AI agent: [date of birth muted]

0:55 Representative: Okay, he hasn't been to this office yet. What location is it?

1:02 AI agent: The Van Ness.

1:05 Representative: Okay.

1:07 AI agent: Yeah, that's 1005 Van Ness Avenue.

1:30 Representative: So for Van Ness, it says 1,855 for the Cleerly exam.

1:37 AI agent: Okay, is that the total price, including the scan and the analysis, or are there separate fees?

1:44 Representative: No, that's all together.

1:47 AI agent: Perfect. Okay, great. And is there anything else like radiologist fee, contrast, anything like that?

1:56 Representative: No, everything is included in that price.

1:58 AI agent: Great, okay. And do you happen to have anything in the afternoon of Tuesday, December 1st, say between like 1:30 and 5?

2:07 Representative: We would need the order so they create an account for him because he is a new patient. So once the order comes in, they automatically create an account for him, and then I can schedule.

2:20 AI agent: Okay, gotcha. So his primary care doctor is in [location muted]. Can they send the order over, or do you have your own ordering physician or a self-pay program?

2:30 Representative: No, they can send it over via fax, and once they send it to us, they automatically create the account.

2:42 AI agent: Okay, great. Is there anything the order needs to say specifically, like wording or a CPT code?

2:50 Representative: Let me see for the CCTA. Because it would be a diagnosis from the doctor, so we really don't do the CPT code. It would be whatever he's feeling, the doctor will put it in the order. But let me see the CPT. It would be 75574.

3:20 AI agent: Okay, perfect. And if they have any openings Tuesday afternoon, Thursday morning, December 3rd, that would work as well.

3:31 Representative: Yeah, and it would be the self-pay, so he'll be able to get in right away. Also, the doctor would have to give him metoprolol, 50 milligrams, because his heart rate has to be 60 or under in order to get a clear read.

3:49 AI agent: Okay, and is there a pre-scan consult needed, and could that be done by video now in November?

3:57 Representative: No, there's no pre-scan needed. Just, once we have the order, we'll schedule the exam, and then all he needs is the medication and no food four hours, no caffeine six hours, drink plenty of water, and arrive 30 minutes prior to the exam.

4:15 AI agent: Perfect, that's very clear, thank you. And just so I have it, what's the best fax number for his doctor to send the order to?

4:23 Representative: Of course, let me give you the fax. It would be 571... 571... 505... 5... 480... 614...

4:57 AI agent: Sorry, I just want to make sure I got that. 571, 571, 505...

5:05 Representative: 5055.

5:08 AI agent: Okay, so 571, 571, 5055. Go ahead.

5:16 Representative: And then it's 480, 614, 8666.

5:38 Representative: Hello?

5:41 AI agent: Yeah, so just to make sure I have that right: the fax is 571, 571, 5055, 480, 614?

5:55 Representative: 8666.

5:57 AI agent: Oh, okay, got it. So 571, 571, 5055, 480, 8666.

6:05 Representative: No, there's two fax numbers. 571, 571, 5055. And the second fax number is 480, 614, 8666.

6:21 AI agent: Oh, that's perfect. Thank you for clarifying that. And just so I have your name, who am I speaking with?

6:28 Representative: [staff name and adjacent words muted]

6:31 AI agent: Great, thanks, [staff name muted]. You've been super helpful. We'll get that order faxed over and [customer name muted] will call back to confirm once the account's set up. Appreciate it, bye!

6:42 Representative: Bye.

2:28 Representative: Thank you. Can you provide me the phone number? I got [phone digits muted].

2:34 AI agent: Sure! [phone number muted]

2:46 Representative: Thank you so much. By any chance, do you have the email address for the patient?

2:52 AI agent: Yes, it's [email address muted].

3:12 Representative: Okay, thank you. And would this appointment be with or without insurance?

3:18 AI agent: It'll be self-pay, so without insurance.

3:22 Representative: Understood. Thank you so much. One moment.

3:44 Representative: And what would be the first and last name for the doctor?

3:49 AI agent: First name [doctor name muted], last name [doctor name and spelling muted].

4:00 Representative: Thank you. Can you provide me with the mailing address for the doctor's office?

4:05 AI agent: Mm-hmm. It's [doctor's office address muted].

4:19 Representative: Thank you, one moment.

[Waiting while the representative works, about 4:21 to 6:32.]

6:32 Representative: By any chance, is there any specific facility to book your appointment?

6:38 AI agent: Yes, the Van Ness, San Francisco location, please.

6:47 Representative: Thank you. And is there any specific date to schedule the appointment?

6:52 AI agent: Yes, [customer name muted] can do Tuesday, December 1st afternoon or Thursday, December 3rd morning, if there's availability.

7:05 Representative: Understood. Thank you so much. One moment, please.

[Waiting while the representative works, about 7:09 to 8:40.]

8:40 Representative: For December 3rd, I do see that the earliest appointment is at 9 AM. Would that work?

8:49 AI agent: Is there any availability on Tuesday, December 1st?

8:57 Representative: Yes, of course. And you told me that would be in the afternoon. Is that correct?

9:02 AI agent: Yes, that's right!

9:05 Representative: I see that in the afternoon it would be at 12:30 PM, or also 1, 1:30.

9:15 AI agent: 1:30 would be best for him.

9:18 Representative: Mm-hmm. No problem. By any chance, can you provide me with a diagnostic code?

9:27 AI agent: Yes, it's ICD-10 [diagnosis codes muted].

9:41 Representative: Understood. Thank you so much. And as for the screening questions, would it be okay to ask them to you, or would you like that we call the patient directly?

9:58 AI agent: Calling him directly would be best.

10:01 Representative: Of course, no problem.

10:20 Representative: So, the appointment has been scheduled for December 1st at 1:30 PM in SimonMed Van Ness.

10:30 AI agent: Perfect!

10:32 Representative: And can I assist you with anything else for today?

10:35 AI agent: Just where can I send the signed order so you have it on file?

10:40 Representative: Yes, would you like a fax number?

10:44 AI agent: Yes, that's great. Thank you.

10:46 Representative: That would be 602...

3:29 AI agent: By email, please.

3:33 Representative: By email. Okay. And is it okay if I ask, does the patient have any insurance?

3:55 AI agent: He's self-pay.

3:58 Representative: Okay, thank you. Okay, so what would the patient need to confirm with the appointment, or is it to schedule an appointment?

4:17 AI agent: There is a scheduled appointment already. It's on Tuesday, December 1st, 2026, at 1:30 PM Pacific.

4:28 Representative: Okay. Okay, I see the scheduled appointment here, and what is it that needs to be confirmed here?

4:38 AI agent: [staff name muted] called this morning asking for [customer name muted]'s height and weight, and some pre-screening questions. Is that what you're looking for?

4:50 Representative: Oh, let me just double check it here.

4:54 AI agent: Sure.

5:17 Representative: Okay, so there are a few information here that the patient is missing.

5:25 AI agent: Okay.

5:27 Representative: We'll just need the patient's height and weight, and if he has done any prior procedures.

5:36 AI agent: Yeah, his height is [height muted], and his weight is [weight muted]. Can you read those back to make sure they're right?

5:50 Representative: Okay, so the patient's height is [height muted] and his weight is [weight muted]. Is that correct?

5:59 AI agent: [height and weight correction muted]

6:10 Representative: That's what I said. [height and weight muted]. Okay.

6:17 AI agent: Great, thanks.

6:18 Representative: And can you verify the... Sorry.

6:24 AI agent: As far as prior procedures, I don't have that one. [customer name muted] will confirm it himself.

6:36 Representative: Hmm, okay. Do you know if the patient has had kidney issues or failure?

6:47 AI agent: I don't have that one. [customer name muted] will confirm it himself.

6:59 Representative: Okay. Well, this is the only information that you can answer for me. So, let me see here what else.

7:32 Representative: Okay, so there doesn't seem to be anything else I need to get asked from you. Okay. If there's anything else that you want to ask me, please feel free to do so.

7:45 AI agent: Sure. Since he'll confirm the rest himself, can you email the screening form to me at [email address muted], or text it? Or can he just do it at check-in? Is there a deadline for that?

7:58 Representative: Yes, you can... You can get it on the day of the service.

8:05 AI agent: Great. Is Dr. [doctor name muted]'s order attached to the appointment?

8:14 Representative: Let me see here.

8:23 Representative: What is the name of the patient's doctor?

8:26 AI agent: Dr. [doctor name muted].

8:41 Representative: Yes, both documents are here in the appointment schedule.

8:49 AI agent: Great, thanks! And when exactly should he take the metoprolol? And are there food, drink, or water rules, especially caffeine? Is it six or twelve hours before? And what time should he arrive?

9:03 Representative: The patient should arrive at least fifteen minutes early before their procedure. No, actually, no. Thirty minutes before their appointment time, so preparations can be verified. The patient should not ingest caffeine six hours prior to their exam. This includes coffee, soda, energy drinks, tea, chocolate, and anything else that may contain caffeine. The goal here is to maintain a low heart rate.

9:35 AI agent: Okay, that's good to know, thank you. And when should he take the metoprolol?

9:47 Representative: What medication, sorry?

9:51 AI agent: Metoprolol.

9:58 Representative: Let me check here.

[Waiting while the representative checks, about 10:00 to 11:10.]

11:11 Representative: Okay, sorry for the wait. I had to double check, but the patient should take it just two hours before.

11:18 AI agent: Perfect. Thank you. I think that's everything for now. Thanks again. Bye!

11:23 Representative: Okay, thank you.

1:44 Representative: Thank you for calling SimonMed. My name is [staff name muted], and how can I help you today?

1:50 AI agent: Hi, this is [assistant name muted], [customer name muted]'s assistant. This call may be recorded. I called earlier today about his heart scan on December 1st.

2:02 Representative: Oh, okay, yeah, let me double check that for you. But before we proceed, can you please verify your complete name as well as your date of birth?

2:14 AI agent: Sure, his name's [customer name muted]. He goes by [customer name muted]. Date of birth is [date of birth muted].

2:24 Representative: Thank you so much. Give me one second.

2:31 Representative: [customer name muted]. Can you spell the complete name of the patient?

2:38 AI agent: Yeah, first name [spelling muted], last name [spelling muted].

2:48 Representative: And what about the phone number of the patient?

2:52 AI agent: Hmm, [customer name muted] doesn't have a phone, but you can reach me, [assistant name and phone number muted].

2:58 Representative: [phone number read back, muted]

3:16 AI agent: That's right.

3:19 Representative: Your name is [assistant name muted], right?

3:21 AI agent: Yes.

3:23 Representative: And how are you related with [customer name muted]? Are you a provider?

3:32 AI agent: No, I'm [customer name muted]'s assistant.

3:36 Representative: Oh, assistant.

3:38 AI agent: Yep.

3:42 Representative: Give me one second, let me pull up the account of the patient, okay?

3:47 AI agent: Sure, no problem.

3:48 Representative: Thank you so much. [date of birth muted]

4:08 AI agent: Yep.

4:17 Representative: [customer surname muted], I'm still working on it, okay? Just stay on the line.

4:21 AI agent: Sure thing.

4:37 Representative: Can you please verify what is the complete name? Because I can see here that there is like a middle initial of the patient, or a middle name.

4:47 AI agent: Yeah, it's [customer name and spelling muted].

4:56 Representative: Thank you so much. And what about the complete address of the patient?

5:05 AI agent: [address muted]

5:16 Representative: Thank you so much, [assistant name muted]. Yeah, I'm sorry, I have to verify this kind of information for HIPAA compliance, okay?

5:26 AI agent: Oh, no problem.

5:30 Representative: And also, since you don't have like a phone number of the patient, I just want to verify what is the email address associated with the account.

5:41 AI agent: It's still [email address muted].

5:49 Representative: Thank you so much. And yeah, how can I help you today?

5:56 AI agent: Thanks. So, [customer name muted]'s height for the screening was entered wrong. It's [height muted]. His weight, [weight muted], that's correct. Can you update the height to [height muted]?

6:11 Representative: Okay, [height muted].

6:15 AI agent: Sorry, that should be...

6:16 Representative: That's his height, [height muted]?

6:17 AI agent: Yes, sorry, that's [height muted]. And can you read back both height and weight just to confirm?

6:28 Representative: Okay. Height is [height muted], and he is [weight muted].

6:36 AI agent: Thanks. Now, can you change his email...

6:39 Representative: Am I correct?

6:40 AI agent: Yes, that's correct. Can you change his email on file to [customer name muted]'s own email? It's [email address and spelling muted].

6:59 Representative: Alright. Yeah. Is it all lowercase?

7:06 AI agent: Yes, [email address muted], all lowercase. Can you read it back letter by letter so we know it's correct?

7:15 Representative: Okay, it's [email address read back letter by letter, muted].

7:42 AI agent: That's correct. Will you email the remaining screening questions to that address?

7:49 Representative: Hm, we will double check that later. So right now, I am going to, you said, to change the screening question of the patient, right?

8:03 AI agent: Just to email the remaining questions to [customer name muted] at that new email. So then he can get those answered.

8:13 Representative: Give me one second.

8:16 AI agent: Sure thing.

8:18 Representative: What about, I am going to ask, you know, some of the screening questions, so I can take a note on that as well on our end. Is it possible that...

8:35 AI agent: Oh, [customer name muted] will answer those himself by email or at check-in. So could you send those questions to that email?

8:48 Representative: Let me double check that for you.

8:52 AI agent: Okay.

8:53 Representative: Give me one second. Okay. He is [height and weight muted].

9:05 AI agent: That's right.

9:09 Representative: Is it possible to put on hold for a minute or two?

9:12 AI agent: Sure, no problem.

9:14 Representative: Thanks. Thank you.

[Hold, about 9:15 to 12:41.]

12:42 Representative: Hello, [assistant name muted]?

12:43 AI agent: Hi.

12:46 Representative: Oh, thank you for your patience in waiting. I actually already changed the height and the weight of the patient on the screening questions. And unfortunately, we can't, you know, we can't able to send you an email for that. But I can answer the questions for you either, if you want it.

13:15 AI agent: Oh, [customer name muted] will answer those himself at check-in then. So, can you just make sure his height is [height muted] and his weight is [weight muted], and that [email address muted] is on file in case you email him something else.

13:34 Representative: Yeah, I already changed that for you. Again, I already changed that before. The height of the patient on the screening questions, it's [height muted]. Now it's already updated into [height muted]. And the weight of the patient is already, you know, it's already [weight muted] here on our end.

14:05 AI agent: Thanks. Yeah. And is [customer name muted]'s email on file [email address muted]?

14:12 Representative: Yes, you are correct, because I already changed that as well.

14:17 AI agent: Perfect. Thanks. Okay, that's all I needed. I appreciate your help. Bye.

14:24 Representative: You're welcome. Is there anything else?

Have your agent make this call

Connect call4me to your assistant through the Claude Code guide or the Codex guide. This is a suggested brief, not the one the customer used:

Call SimonMed cardiology scheduling at 1-844-615-2957 about a self-pay coronary CT angiogram with AI plaque analysis at [location]. Ask the total self-pay price and whether it includes the analysis, radiologist fee and contrast. Ask which fax number my doctor should send the order to and read it back. Once the order is sent, call again and ask whether it's attached to the appointment, and have them read the exam name exactly as entered. Book [date and time window] only after I confirm. Ask the arrival time and the prep rules for food, water, caffeine and heart rate medication, and repeat each back. Give my name, date of birth, address and email only when they ask to verify. Don't give any card number.
